Qualifying Life Event

Need to Make Changes Throughout the Year?

The choices you make when you first become eligible remain in effect for the entire plan year ending December 31st. Once enrolled, you must wait until the next annual open enrollment period to make changes to your benefits, UNLESS you experience a QLE (Qualifying Life Event). QLE’s must be reported to the benefits department within 60 days of the day the event took place.

What is a QLE?

Examples of Qualifying Life Events include: marriage, divorce, birth or adoption of a child, gain or loss of other health coverage, or death of a covered dependent.

If you have a life event change, you will need to documentation that includes the affective member/depent and the change effective date. Documentation should be submited to benefits@nsm-seating.com. Below are examples of typical accepted QLE documentation.

Birth of Child

  • Birth certificate or discharge paperwork showing the name of the child and date of birth.
Adoption of Child
  • Paperwork showing the date the adoption took place and the child’s name.
Marriage
  • Marriage certificate showing the date the marriage took place.
Divorce
  • Divorce decree or court documents showing the date the divorce took place.
Loss/Gain of Coverage Elsewhere
  • Documentation showing names of those impacted and the effective drop / start date. If adding coverage, date of birth and social security number will also be required to add the member to the benefits system.

  BCBS Network & Finding a Doctor

Go to bcbst.com/findadoctor. Include your network and geographical information. Browse by category or search by provider name or specialty. Narrow your results by using filters at the top of the search results page. 
  • Network (in-state, residing in TN) - Network P
  • Network (all states other than TN) - BlueCard PPO
Please Note: All BCBS ID cards show Blue Network P. However, if you live outside of the state of Tennessee, use the BlueCard PPO network to search for your provider. 


  Teladoc

You can get started using online video chat or the free BCBSTN app. Just have your Member ID card ready. Register by logging on at www.bcbst.com/Teladoc and choose "Talk With a Doctor Now". You can also call 1-800-TELADOC (1-800-835-2362).

The first time you use Teladoc Health, you'll need to fill out a short medical history surevey and create an account. The next time you use it, you can just log in through the app and talk to a doctor in minutes.

    • General Medical Care: $55 per visit
    • Dermatology: $85 per visit
    • Nutrition Counseling: $59 per visit
    • Therapist: $90 per visit
    • Psychiatrist: $220 initial visit and $100 for subsequent visits
