2024 Form 1095-Cs

As a part of the Affordable Care Act, the IRS requires the distribution of a document called a “1095” by March 3, 2025. The 1095-C is one of three versions of the Form 1095. This version is distributed by NSM to all full-time employees. The form indicates if you were offered a medical plan that met both minimum value and affordability requirements and also confirms each month of enrollment during the 2024 plan year.


Form 1095-C to be Mailed

Documents are being mailed to home addresses on file in UKG. We anticipate delivery of these forms the first week of March. If you need a digital copy of this document, please email benefits@nsm-seating.com and the Benefits Team will email you a copy. 


Kaiser CA

For those employees who were enrolled in a Kaiser CA medical plan during 2024, you will also receive a 1095-B directly from the insurance carrier, indicating any months of coverage for you and your covered dependents.


Marketplace Enrollees

If you purchased coverage through the Health Insurance Marketplace during 2024, you will receive a 1095-A. 

Copies of this document will also be provided to the IRS by NSM and/or the insurance carrier. Be sure to keep this form for your tax records!

Updated SAR & SMM Documents Posted


Summary Annual Reports (SAR) are required by the Employee Retirement Income Security Act of 1974 (ERISA). New SARs for both the 401k Plan and the NSM Benefit Plans have been posted on WNSM for your review. These documents include a summary of the employee benefit plan’s financial information. 

Summary of Material Modifications (SMM) documents have also been posted for both the 401k Plan and the NSM BlueCross BlueShield medical plan. Click the link for Plan Documents on the Benefits Page of WNSM to learn more.
